"With a Gun in My Hand" is the 4th episode of Lupin the 3rd Part 4.
Official Synopsis[]
A lonely town. Jigen, who has aching cavities, irresistibly goes to the hospital. Instead of seeing him, Dr. Levia asks him to leave his gun. Unfortunately, he has no choice but to hand over the gun. Waiting for Levia's consultation, Jigen witnesses a myriad of patients lying dead in a hospital room. They are called "living corpses" and are all the consequences of a gangster boss named Erik. In this town, you can't even have a gun without Erik's permission. At that time, Erik appears in the hospital.

- In the hospital where Jigen went to, some of the patients resemble other characters due to reusing character designs from Lupin the 3rd Part 2:
- The man with a grey mustache wearing a suit standing up looks like Professor Lonebach,
- The woman sat down with the beret looks like Mrs Dokonjo
- Behind the woman is a man who resembles Yasuo Otsuka who works for Telecom and previously had cameos in Lupin the 3rd Part 1. At an earlier scene, Yasuo Otsuka is stood next to the man with the grey mustache having a drink.
